Issue of Ordinary Passport (36 pages) in case of minor (below 15 years)

Duly completed online registered Passport Application Form to be submitted with original passport, copy of the passport of minor (if any) and self-attested copy of passport of parents(first two and last two pages and the page of observation (if any), made by Passport Issuing Authority  and current visa page or valid work/residence permit) and two recent coloured photographs in white background of size 5cm x 5cm. Issue of Passport in lieu of lost, damaged or stolen passport:

Duly completed online registered Passport Application Form to be submitted with original passport (in case of damaged passport), copy of the passport (first two and last two pages, and the page of observation (if any), made by Passport Issuing Authority  and current visa page or valid work/residence permit)and two recent coloured photographs in white background of size 5cm x 5cm. Police Report and self-declaration of the applicant explaining the loss/theft/damage of the passport will also be required. Please see the Annexure F as a Specimen for making self- declaration :

Applicants applying for re-issue of Passport to add/delete his/her spouse name;  woman applying for change of surname after marriage; divorcee applying for change or deletion of spouse name in existing passport; or re-married applicants applying for change of spouse name in existing passports.

Duly completed online registered Passport Application Form to be submitted with original passport, copy of the passport (first page, last page and current visa page or valid work/residence permit) and two recent coloured photographs in white background of size 5cm x 5cm.  In addition to above, supporting documents in respect of change of particulars is also required.

Re-issue of passport for change of name due to reasons other than given in column  (4)

Duly completed online registered Passport Application Form to be submitted with original passport, copy of the passport (first page, last page and current visa page or valid work/residence permit) and two recent coloured photographs in white background of size 5cm x 5cm and clippings of two local newspapers or the gazette notification of the concerned state government, as the case may be; 
At least two public/school documents issued in the desired /applied change of name to ascertain that the applicant has actually changed his/her name.

Re-issue of Passport due to change of address

Duly completed online registered Passport Application Form to be submitted with original passport, copy of the passport (first page, last page and current visa page or valid work/residence permit) and two recent passport size coloured photographs with white background. and 
Water/Telephone (landline or post- paid mobile bill)/Electricity bill/Statement of running bank account (Scheduled Commercial bank excluding Regional Rural banks and local area banks)/Income Tax Assessment Order/Election Commission Photo Id card/Gas connection bill/Certificate from Employer of reputed and widely known companies (only public limited company) on letter head/Spouse's passport copy (Pages mentioning personal particulars including family details provided the applicant's present address matches the address mentioned in the spouse's passport)/Parent's passport copy, in case of minors (Pages mentioning personal particulars)/Aadhaar Card/ Registered Rent Agreement.

Emergency Certificate

Visit https://embassy.passportindia.gov.in  create a user id after registration and apply for Emergency Certificate online. A pdf form as Emergency Certificate application will be generated after successfully filling up of your details. Take a print out of the application and submit it at the Consulate along with following documents :

1. Copy of the passport (first page, last page and current visa page
2. Two recent coloured photographs with white background of size 3cm x 3cm and two similar photographs of size 5cm x 5cm.
3. Police Report
4. Any other document(s) in proof of identity.

1. Self-declaration of the applicant explaining the loss/theft/damage of the passport and confirmed air ticket. Please see Annexure F as a specimen for preparing self-declaration.

Police Clearance Certificate

The procedure for applying PCC application is as follows :
Visit https://embassy.passportindia.gov.in  create a user id after registration and apply for PCC online. A pdf form as PCC application will be generated after successfully filling up of your details. Take a print out of the application and submit it at the Consulate along with following documents :

1. Original passport (for verification)
2. Copy of passport (first page, last page and current visa page or valid work/resident permit)
3. Two recent coloured photographs in white background of size 5cm x 5cm. Paste one photograph on the application form generated online.
4. Copy of employment contract/ offer letter by the company

In view of recent change in policy of Government of India relating to issue of PCCs, Consulate shall issue PCC only after receipt of fresh Police Verification Report on the PCC application from the police station concerned.  PCC will not be issued in case of receipt of any negative remarks from the police authorities.  Applicants are advised to apply well ahead of time (at least one month) to enable us provide timely service.
Kindly note that PCC applications will not be accepted from Tourist Visa holders.
