Issue of Ordinary Passport (36/60 pages) |
i) Duly filled application form. Applicant should give attention while filling up the form that all particulars are correct and true. ii) If there are no changes in particulars from the last Passport- self attested copies of the first and last pages of passport, self-attested copies of resident permit/work permit or copy of long term Visa. (In case Work Permit/TRC is getting expired or has expired, copy of the latest visa issued) iii) In case of changes in particulars from the last passport, supportive documents must be attached, like marriage certificate for spouse name, Aadhar Card or any other address proof for address change etc. If any document issued in Vietnams (like marriage certificate), it must have been attested by the consulate. |

Issue of Ordinary Passport (36 pages) in case of minor (below 15 years) |
i) Duly filled application form. Applicant should give attention while filling up the form that all particulars are correct and true. ii) In case of fresh Passport for minor, Birth Certificate issued by Consulate General of India, Ho Chi Minh City or Embassy of India, Hanoi; copies of Passports of parents, TRC/Work Permit of Parents, Marriage Certificate of Parents, Annexure D or C (Whichever applicable), ii) In case of re-issue of Passport for minor, copies of last Passport and TRC/Visa of minor, copies of Passports of parents, TRC/Work Permit of Parents, Marriage Certificate of Parents, Annexure D or C (Whichever applicable), |

Applicants applying for re-issue of Passport to add/delete his/her spouse name; woman applying for change of surname after marriage; divorcee applying for change or deletion of spouse name in existing passport; or re-married applicants applying for change of spouse name in existing passports. |
Duly completed online registered Passport Application Form to be submitted with original passport along with documents mentioned in point 1 (ii) of the table. |

Issue of Passport in lieu of lost, damaged or stolen passport for TRC/Work Permit holders: |
Duly completed online registered Passport Application Form to be submitted with original passport (in case of damaged passport). Copy of the passport in case of lost/stolen passport (first two and last two pages, and the page of observation (if any)), made by Passport Issuing Authority and current visa page or valid work/residence permit); Police Report and self-declaration of the applicant explaining the loss/theft/damage of the passport will also be required: All documents must be upload while applying Online. Re-issue of passport for change of name due to reasons other than given in column (4) |
Duly completed online registered Passport Application Form to be submitted with original passport. copy of the passport (first page, last page and current visa page or valid work/residence permit) and clippings of two local newspapers or the gazette notification of the concerned state government, as the case may be; at least two public/school documents issued in the desired /applied change of name to ascertain that the applicant has actually changed his/her name must be uploaded while applying online. |

Re-issue of Passport due to change of address |
Duly completed online registered Passport Application Form to be submitted with original passport, copy of the passport (first page, last page and current visa page or valid work/residence permit) and one of the above documents may be uploaded as proof of change of address. |

Emergency Certificate for Tourist Visa/Short Term Visa holder |
Visit https://mportal.passportindia.gov.in/gpsp create a user id after registration and apply for Emergency Certificate online. A pdf form as Emergency Certificate application will be generated after successfully filling up of your details. Take a print out of the application and submit it at the Consulate. Following are the documents which have to be uploaded while filling the form online:

Police Clearance Certificate |
The procedure for applying PCC application is as follows :
In view of recent change in policy of Government of India relating to issue of PCCs, Consulate shall issue PCC only after receipt of fresh Police Verification Report on the PCC application from the police station concerned. PCC will not be issued in case of receipt of any negative remarks from the police authorities. Applicants are advised to apply well ahead of time (at least one month) to enable us provide timely service. |

Online appointment
It is mandatory to have online appointment for consular services including visa, passport, miscellaneous services on Passport, attestation and consular services and OCI. The online appointment must be booked at least one day in advance. The web-link for booking on line appointment is:

Please note that Consulate does not send any e-mail to applicants to confirm their appointment. Appointment is considered to be confirmed once the applicant has booked the time slot successfully.
